Dr. Zembroski offers long-distance telephone appointments, providing initial consultations, health evaluations, second opinions, and ongoing care for individuals who have not achieved a resolution to their health problems.

For those of you who live at a distance, where traveling to see me is not convenient, I offer a long-distance consultation which allows you to discuss your health condition with me. Due to the vast information and research available, I am able to draw from an extensive database, which will help me get to the reason behind your ill health.

Once I have reviewed all of your history and health information, I will then discuss the findings with you by phone. At this time we form a partnership, one where we will work together to restore your health in the shortest time possible.

You can rest assured knowing that you will be heard and listened to. It is important that you express any and all facts and bits of information about yourself, anything that will help me to help you to get to the root of your issue.

Every step will be explained in detail. If, after my assessment, I realize I cannot be of service to you, I will let you know immediately and make recommendations or discuss other alternatives that may be available to you. If you are currently under another doctor’s care, my program is designed to work with any existing treatment/protocol that you may be doing.

There are 5 steps to your phone consultation:

The first step is to complete the Functional Medicine Assessment, which can be found under the “Patient Forms” under the Darien Functional Medicine link in the menu bar.

Unlike most medical forms, the Functional Medicine Assessment is designed to uncover the root causes of your health issues.

It is very detailed.

The details are important, as even the smallest clues can help to determine why you developed your health problem.

All copies of your medical records, blood tests, diagnostic testing, etc., are needed.

You can obtain and send the records yourself, or we can help you with this step. If you would like us to do it, we need a signed medical release form.

Once we have that, we can contact your physician(s) to have them fax or email your records to our office.

Many medical offices now charge a nominal fee to send your medical records, so it may be easier or more cost-effective for you to obtain them yourself.

After all of your personal and medical information has been reviewed, a member of our staff will contact you to schedule the first phone consultation.

During our conversation, I will review and discuss what was found, and what those objective tests mean. If I recommend additional testing, I will fully explain why this testing is needed.

Usually, the initial consultation time is about 45 minutes.

It’s important that you ask any questions you may have. This is a “no judgment” time; discussing your personal health history is an important step in getting to the root of your issue and creating an action plan to get it resolved.

In my opinion, many practitioners don’t take the time to fully discuss your health concerns. The seven-minute visit does not work.

You and I will take ample time to discuss all of your concerns and goals during our conversation. Many times, the solution to your health issue is uncovered just through questioning and listening.

If additional diagnostic testing is required, I will discuss the reason and cost for those test(s). Blood work can be performed at a lab (Quest or Labcorp) located close to you.

Other functional tests (hormones, organic acids, toxicity, etc.) can be performed in the convenience of your home, using home-testing kits we can send to you.

Any test requiring your local physician will be discussed with you, and arrangements will be made.

After embarking on your individualized program, follow-up consultations will be needed, and may involve repeat or additional testing, as well as modifications to your treatment program.

In between consultations, I suggest you keep an honest, personal journal of your progress. Points you may include, aside from how you are feeling, are what you have been eating for the day (including beverages), your consistency with your personalized treatment program, any questions you may want to ask (highlight those so you remember when we speak next), and anything else regarding your life.

The journal is a tool to be utilized for your health benefit.

Charges and Payment

Payment is due at the time services or products are rendered, via cash, check, Visa, MasterCard, American Express or Discover. My office participates with CareCredit, a healthcare credit card offering interest-free plans. For more information on CareCredit, please call my office. We do not participate with any health-insurance plans.

However, if you have out-of-network benefits, your plan may pay some of our services, as well as some of the specialized testing that you may undergo. We will be happy to assist you in obtaining insurance reimbursement. It may be a good idea to call your insurance carrier in order to better understand your policy and its benefits.
